Center City Housing is in need of a part-time Registered or Licensed Practical Nurse in our Silver Creek Corner Supportive Housing Facility in Rochester, MN. The RN or LPN is responsible for carrying out the mission of CCHC by providing services needed to support the resident health needs in a supportive housing apartment building serving formerly homeless single adults and/or chronic inebriates. This position involves working with a team of employees, volunteers, and partners to carry out the program goals based on “Harm Reduction” and “Housing First” philosophies.
The position needs a unique combination of nursing and social service skills. This role requires a person who is self-motivated with the ability to work independently and as a member of a team to carry out job duties. Supportive Housing Development Director
Full-time salaried position
The Supportive Housing Development Director supports the mission of CCHC by working with communities and partners to develop new housing consistent with CCHC’s mission and “Housing First” philosophy for supportive and affordable housing. The role is responsible for new housing development activities, securing funding for capital, supportive services, and operations in new and existing housing projects. This includes Federal, State, local and philanthropic funding. This position requires regular out-of-town, overnight travel.
This position requires a unique combination of development experience and social service skills. A degree or equivalent experience in multi-family development, knowledge of finance tools for affordable housing development, rental subsidies, and supportive services funding, ability to develop and manage budgets, program management, fund development, subsidized property management, low-income real estate development or related fields is required. This position requires a valid driver’s license and dependable transportation. This person must pass and maintain Minnesota Background Study clearance.

Supportive Housing Youth Case Manager (Rochester, MN)
Center City Housing is looking for a Case Manager to serve youth from age 16 to 22 in our Gage East Supportive Housing Facility in Rochester, MN.
Overview:
Provide case management, advocacy, and support services to homeless individuals who are participating in the Gage East supportive youth housing program. Ensure program success by supporting individuals to create long-term goals, weekly goals, and objectives that lead to improved skills/education/income and movement to permanent housing. Provide information and referrals, home visits, community meetings/programs and advocacy for participants with community providers of services. Maintain appropriate recordkeeping and client records in accordance with federal, state and Center City Housing Corporation requirements. Work closely with Site Director, Case Managers, property manager, and other community agencies.
Preferred experience includes a bachelor’s degree in human service field preferably with two years experience in case management for homeless people/youths OR Associate Degree in human service field with three years experience in case management for homeless people/youths. Center City Housing Corp. is looking for candidates to fill our full-time position in Rochester, MN as Early Childhood Program Coordinator for Gage East Apartments.
This position works closely with the Supportive Housing Case Managers to provide comprehensive services to parents/guardians and their children living in transitional or permanent supportive housing. The Early Childhood Program Coordinator provides programming on site for children from birth through four years old, facilitates parent/guardian groups addressing the needs of children, and refers to community services. As a member of the service team, the Early Childhood Program Coordinator works in partnership with the Case Managers to provide whole family supportive services. In addition to other duties, the Early Childhood Program Coordinator provides supervision and direction to the TOTs program staff. The person in this role will work closely with community agencies to develop positive relationships. This position also works with the Site Director to implement grant-funded activities and complete programming evaluation.

Part-Time Early Childhood Program Assistant (Duluth, MN)
Center City Housing is in search of an Early Childhood Program Assistant for our Family Supportive Housing Facilities in Duluth, MN.
This position works closely with the Early Childhood Program Coordinator to provide comprehensive services to children from birth through four years old living in supportive housing. The Early Childhood Program Assistant will assist the Early Childhood Program Coordinator with both long- and short-range children’s activities in accordance with program objectives, developmentally appropriate practice, and program philosophy. The Early Childhood Program Assistant works with children one-on-one or in a group.
This position requires a minimum of one year experience working with low-income and homeless children and families. This person must pass and maintain Minnesota Background Study clearance.
